Population Health

Population Health

Collaborative and compassionate care teams help drive Somatus’ mission forward throughout the country. Our multi-disciplinary care teams are typically comprised of various roles from front-line clinicians to field operations leadership.

Check out the key responsibilities of three of our care team roles.

Registered Nurse Care Manager

The RN Care Manager serves as the Captain of our Care Team, directing all care team specialties. Key responsibilities include:

  • Conduct comprehensive care assessments and in-home evaluations
  • Help patients establish healthcare goals
  • Coordinate patient care team plan across all provider disciplines
  • Develop and communicate patient care plan with patient and providers
  • Prevent clinically avoidable hospital admissions/readmissions
  • Manage patient comorbidities
  • Educate and empower patients to take an active role in their health

Patient Health Advocate

Working closely with the RN Care Manager, our Patient Health Advocates:

  • Assess patient home environments (identifying food insecurity, falling hazards, etc.)
  • Drive progress towards care plan goals
  • Provide caregiver support
  • Coordinate care (make appointments, arrange DME)
  • Address social barriers (e.g. access to transportation)
  • Connect patients to community resources
  • Support with hospital post-discharge activities to prevent returns to the hospital

Clinical Operations Manager

Our care teams report to a Clinical Operations Manager who:

  • Leads care teams in achieving operational and clinical goals to keep our patients healthy at home.
  • Reviews, manage and coordinate all care team assignments.
  • Serves as first line of defense to achieve clinical and operational targets
  • Identifies areas of opportunity and coordinate solutions to optimize outcomes
  • Manages staff adherence to Somatus workflows, protocols, best practices, and documentation expectations
  • Empowers care teams to advance their learning and growth and development within the organization
Inpatient and Outpatient Dialysis

Inpatient and Outpatient Dialysis

Somatus helps dialysis patients navigate their health in two unique settings: Our state-of-the-art comprehensive kidney care centers in Falls Church and Mount Vernon, Virginia and as a healthcare partner of Inova Health System. Our inpatient and outpatient dialysis teams work collaboratively with patients and the healthcare system to explore the right dialysis environment for them (home or in- center) as we help patients navigate the complicated transplant process. Our transplant rate is 10.86 (per 100 patient years) which is notably 3.8 times above the national average.

Corporate Operations

Our patient-facing teams are able to create more healthy days with the support of Somatus’ corporate operations which houses the functions necessary to deliver on our mission. These our include Business Development, Clinical Operations, Corporate Initiatives, Executive Leadership, Finance and Accounting, Human Resources, Information Technology and Analytics, Legal, Marketing, Medical Advisory, Product Development, Provider Engagement, and Sales and Marketing.
