Our Story
Somatus was founded in 2016 to introduce value-based kidney care. We quickly became the industry leader, driving real and meaningful change for patients, providers, and health plans and will manage more than 160,000 patients in 2024.
Our innovative and comprehensive care model uses AI-driven technology and machine learning to address patient care needs related to medication management, care coordination, nutritional support, and transitions of care support that empowers patients to take an active role in their health. Our multi-disciplinary care teams directly provide safety net care as a partner with our health plan partners and network providers. Through our model, we are able to drive greater health equity and access while managing the full range of patient chronic conditions and needs related to social determinants of health (SDOH).
